INYECCIONES PERINEURALES SUBCUTÁNEAS
Perineural Injection Therapy (PIT)
Conocida como Teparia Neural, actualmente la American Association of Orthopaedic Medicine (AAOM), la denomina inyecciones perineurales subcutáneas (IPS).
A diferencia de muchos tratamientos médicos que proporcionan beneficios temporales, este tratamiento seguro y probado ofrece un alivio duradero.
MECANISMO DE ACCIÓN
Esta técnica consiste en inyectar con una aguja muy fina dextrosa al 5% (D5%) en forma subcutánea a zonas específicas (áreas de deterioro crónico de la fibra nerviosa) sobre el recorrido de los nervios periféricos. El objetivo es mitigar el dolor ocasionado por la inflamación de dichos nervios (dolor neuropático).
La dextrosa al 5% inhibe los receptores vinculados al dolor situado en el nervio (específicamente en el terminal presináptico) denominados TRVP-1 (transient receptor potentia vanilloid 1)
¿EN QUÉ SITUACIONES SE APLICA?
Otorga muy buenos resultados como parte del tratamiento de dolores neuropáticos (polineuropatía diabética, neuralgias postherpéticas, síndrome del túnel carpiano, neuroma de Morton, etc.).
Resulta muy efectivo cuando se lo utiliza como tratamiento para dolores musculoesqueléticos de origen miofascial, síndrome de dolor crónico regional complejo (SDCRC) y fundamentalmente útil dolor cervical y de cintura (lumbalgias, “ciatalgias”), dolores de hombros, fibromialgias, etc.
¿QUÉ PUEDO ESPERAR DESPUÉS DEL PROCEDIMIENTO?
Muchas personas informan un alivio inmediato del dolor en un plazo de 15-30 minutos después del procedimiento. Sin embargo, este no es el caso en todos los pacientes y ocasionalmente puede experimentar una reducción del dolor 1-2 días después del procedimiento. El alivio del dolor después del primer procedimiento generalmente dura días y cuando el dolor regresa, lo hace con mucha menor intensidad. Aproximadamnete luego de 1/2 semanas se administrará la segunda sesión de tratamiento.
Un curso de tratamiento típico consiste en 4-6 sesiones de tratamiento, realizadas con 1/2 semanas de diferencia (dependiendo de la severidad del cuadro). Ocasionalmente, se requieren más sesiones para curar completamente la lesión. El alivio del dolor generalmente dura más y más después de cada sesión de tratamiento sucesiva hasta que finalmente se alivia permanentemente.
BENEFICIOS
- No existen contraindicaciones para este procedimiento. Pacientes diabéticos e hipertensos lo pueden realizar sin riesgo alguno.
- La dextrosa no provoca efectos adversos.
- No requiere preparación previa, ni cuidados posteriores. Puede regresar al trabajo luego de terminada la aplicación.
- Puede efectuar deportes y actividad física el mismo día de la aplicación.
- No existe interacción medicamentosa con otras drogas. Puede continuar con su medicación habitual durante el tratamiento con IPS.
- Las aplicaciones se ejecutan con una aguja tan fina que no provoca dolor.
